How it works
- 1
Add your practice once
Provider details, NPI and tax ID are remembered in this browser, so the next superbill starts half done.
- 2
Enter or import the visits
Pick ICD-10 and CPT codes from the suggestions, or import a CSV from your EHR. Totals and the balance update as you go.
- 3
Print or save as PDF
The checklist flags anything an insurer will ask for. Then print it, save a PDF, or batch one for every client.
What is a superbill?
A superbill is an itemized statement of the care a patient received: who provided it, the diagnosis (ICD-10 codes), each service (CPT codes), dates, place of service and fees. Clients who pay an out-of-network provider attach it to a claim so their insurer can reimburse them, and many also use it for HSA or FSA claims.
Unlike a CMS-1500 claim form, a superbill is not filed by the provider. It is a receipt with the coding an insurer needs. Read thefull guide to superbills, see an annotated example, or learn how clients submit one.
Questions
What is a superbill?
A superbill is an itemized statement from a healthcare provider listing the diagnoses, services, codes and fees for a patient’s visits. Patients who see out-of-network providers submit it to their insurance company to be reimbursed for what they paid.
Is this superbill generator really free?
Yes. There is no account, trial or watermark. The generator runs in your browser and you can print or save as many superbills as you need.
Is my patients’ information private?
The generator runs entirely in your browser. What you type is saved only in this browser’s local storage so it is there next time, and CSV files are read on your device. Nothing is sent to a server. Use “Erase all data” on a shared computer.
Can I import sessions from my EHR or practice management software?
Yes. Export appointments or billing transactions to CSV, then use Import CSV. Columns such as Date of Service, CPT Code, Fee and Client Name are matched automatically, and you can load one client or print a superbill for every client in the file at once.
What does a superbill need to include?
Insurers generally look for the provider’s name, address, NPI and tax ID; the patient’s name and date of birth; at least one ICD-10 diagnosis code; and each service with its date, CPT code, place of service and fee. The checklist above the preview shows what is still missing.
How do I save the superbill as a PDF?
Click “Print / Save PDF” and choose “Save as PDF” as the destination in the print dialog. Pages are laid out for US Letter paper, and only the superbill is printed.
